What is the dual-phase process?

Minder developed the dual phase cognitive evaluation process to increase accessibility and reduce costs for patients. Other neuropsychology clinics require all patients to complete 4-6 hours of neuropsychological testing. However, much of this test data is only useful if the patient is diagnosed with a cognitive disorder. Most people do not have a cognitive disorder. Therefore, patients can save time and money by completing the evaluation in phases. At Minder, all patients complete an online 2-hour cognitive evaluation (Phase 1) and only patients who show evidence of cognitive impairment in Phase 1 are asked to complete the additional 2-3 hours of testing online (Phase 2). What happens during my evaluation?

The Phase 1 testing will take place online through Zoom and will last approximately 2 hours. First, you will complete a clinical interview with one of our expert neuropsychologists, to better understand your history, symptoms, and concerns. Next, you will complete tests that measure a range of cognitive functions, including memory, language, and attention. These tests are non-invasive and will be administered through our online portal using oral and written questions, paper and pencil tests, and other standardized procedures. Most patients will only need to complete Phase 1 testing.

Results will be provided virtually by the neuropsychologist and will include a discussion of test performance, diagnoses (if applicable), and recommendations for improving cognitive health. Patients will also receive a formal written report. For patients who only need Phase 1 testing, results will be provided immediately. For the minority of patients who need to complete additional Phase 2 testing, they will typically receive results within 7 days of the Phase 2 testing appointment.

What is the cost? When do I pay?

Fees for the Phase 1 cognitive evaluation are $1800 and are due at the time of booking. Most people will only need to complete phase 1 testing, so they can save 40-60% by approaching the evaluation in phases. For those who are recommended for Phase 2 testing, an additional $1800 will be due upon scheduling the additional testing appointment.

Are fees refundable? Can I reschedule?

All fees are refundable within 24 hours of purchase, minus any credit card processing fees that may have applied. All appointments can be rescheduled at no cost, as long as you provide 72 hours notice. If you reschedule within the 72 hours window and we are unable to schedule another patient in your place, you will be charged a $199 rescheduling fee.

Does Minder Accept Insurance?

For those with Medicare/Medicaid/AMA Supplemental insurance, Minder offers up to 50% off of costs. The remaining costs after applying the discount are due up front before the first visit. Call us to find out what discount you qualify for.

If you have PPO insurance, Minder is an out-of-network provider. Therefore, many services may be eligible for reimbursement. Generally patients with PPO insurance can expect their insurance to reimburse most costs associated with completing Phase 1 of the evaluation. However, our fees are due up front before the first visit and we cannot communicate with your insurance provider. Your insurance company should be able to tell you how many hours of each CPT code you can expect reimbursement and at what rate. After completing the evaluation, patients will be provided with a SuperBill that includes all of the appropriate CPT codes that were used. Below are the codes you can use to inquire about your out-of-network insurance coverage.

96116 – Neurobehavioral status exam by QHP (1 hour)
96121 – Each additional 30 minutes (1 hour)
96136 –Psychological/neuropsychological test administration/scoring by QHP (30 min)
96137 –Each additional 30 minutes (30 min)
96132 –Neuropsychological Testing Evaluation Services by QHP (1 hour)
96133 –Each additional 30 minutes (1 hour)

Phase 2 Evaluation Codes (*needed for minority of patients):
96138 –Psychological/neuropsychological test administration/scoring by tech (30 min)
96139 –Each additional 30 minutes (5-6 hours)
96132 –Neuropsychological Testing Evaluation Services (1 hour)
96133 –Each additional 30 minutes (3-4 hours)

*All patients will utilize Phase 1 evaluation codes. Only a minority of patients who perform poorly on Phase 1 will be recommended for additional Phase 2 testing. If you have previously been diagnosed with a cognitive disorder, you can reasonably expect to be recommended for Phase 2 testing. For those who complete both, Phase 1 and Phase 2 testing will be reflected on one SuperBill.

Is it typical to pay out-of-pocket costs?

Typically, patients can expect to pay out-of-network costs for a cognitive evaluation, even when using in-network insurance. Most private clinics, like Minder, require payment up front and leave it up to the patient to request reimbursement from their insurance company after services are completed if they have PPO insurance. While you may be able to find a clinic that is in-network with your insurance, most of these clinics also have very long wait-times, as much as nine months and you will likely still incur out-of-pocket costs. It should be noted that Minder's dual-phase process was developed to increase out-of-network reimbursement rates and reduce overall costs for patients.
